Step one

Eligibility — must apply to you

Tick every box that is true for you. All four are required.

I am over 18 years of age. My BMI is 30 or higher, OR my BMI is 27–29.9 with a diagnosed weight-related condition (type 2 diabetes, prediabetes, high blood pressure, high cholesterol, sleep apnoea, fatty liver, or PCOS). I have tried diet and lifestyle changes and understand this medication works alongside — not instead of — them. I have access to GP care for anything outside GLP-1 weight management — my own family GP, or I’ll use Doctor365.ie as a walk-in.

Step two

Absolute exclusions — must not apply

Tick any that apply. If you tick even one, GLP-1 treatment is not suitable.

My BMI is under 27. I am pregnant, breastfeeding, or actively trying to conceive. I have a personal or family history of medullary thyroid cancer or MEN2. I have type 1 diabetes or a history of diabetic ketoacidosis. I have had a previous allergic reaction to tirzepatide or semaglutide. I am currently taking another GLP-1 medication (Ozempic, Saxenda, Rybelsus). I have or have had an eating disorder (anorexia, bulimia, binge eating disorder).

Step three

Cautions — please tick if they apply

These do not automatically exclude you, but Dr Daly will want to discuss them.

I have a history of pancreatitis or gallbladder problems. I have kidney or liver disease. I have diabetic retinopathy or other diabetic eye complications. I take insulin, sulfonylureas, or the oral contraceptive pill. I have significant mental health conditions under active treatment.
