Bunny lines are wrinkles that appear on the bridge of the nose. The lines often become more prominent when smiling and can convey a false appearance of distaste.

  3. The skin overlying the muscles to be injected is marked out with a skin marker.

  4. The injections are placed high up on the nose, to the sides of the bridge of the nose and into the area of maximum wrinkling.

  5. The depth of the injection is placed very superficial, into the skin overlying the Nasalis muscle. The Botox then diffuses down into the Nasalis muscle.

  6. Care must be taken not to inject to laterally (to the sides) or inferiorly (down), to avoid affecting the Levator Labii Superioris Alaeque Nasi muscle that runs down the side of the nose to the upper lip.

  7. The muscles are injected.

  8. It takes about 30 seconds.

  9. Light pressure is applied to the area to prevent bruising.

  10. The skin marker marks are wiped off.

Depressor Anguli Oris. This is the muscle highlighted in the image. The muscle attaches to the corner of the mouth. Excessive muscle tone will pull the corners of the mouth down, resulting in a sad look and also accentuate the so-called Marionette lines.

  3. The injection point is about 1cm above the angle of the jaw, in line with the Depressor Anguli Oris muscle.

  4. Care is taken not to inject too medially (towards the chin), to avoid weakening the Depressor Labii Inferioris muscle.

  5. Care must also be taken not to inject too superiorly (closer to the lip) to avoid weakening the Orbicularis Oris muscle that surround the mouth.

  6. The muscles are injected.

Often the lateral (outside) part of the forehead is involved in isolation. During ageing the brow starts to sag. The forehead then tries to compensate for the saggy brow by contracting to hold the brow up. This contraction of the forehead muscle causes wrinkles on the sides of the forehead.

MUSCLE INVOLVED WHEN DOING A LATERAL BROW LIFT:

  1. The Orbicularis Oculi muscle is the ring shaped muscle around the eye. When the Orbicularis oculi is injected at the spot marked with the blue question mark in the image below, the outside edge of the eyebrow will lift a bit. This is known as a lateral (outside) eyebrow lift. A lateral eyebrow lift is often combined with injections of the brow and forehead.


TECHNIQUE

  • The patient sits back on a bed with the backrest up.

  • The area is cleaned.

  • The skin overlying the muscles to be injected is marked out with a skin marker.

  • The exact placing of the injection points are different between people and depends on the muscle action observed when wrinkling the forehead.

  • Injections of the horizontal forehead lines are always combined with injections of the brow, because injecting the forehead without the brow might lead to heaviness or even slight sagging of the eyebrows.

  • The muscles are injected.

  • It takes about 1-2 minutes.

  • Light pressure is applied to the area to prevent bruising.

  • The skin marker marks are wiped off.


DOSAGE:

Procerus muscle injection (highlighted in the image above) for forehead lines:

  • in females: 10-20 units

  • in males: 16-40 units

Orbicularis oculi muscle injection, marked with the blue question mark in the image above, for a lateral eyebrow lift:

  • in females: 2 units

  • in males: 3-4 units


AFTERCARE:

  • Do not go to sleep or massage the forehead, because this might displace the product toward the upper eyelids, leading to a heavy feeling of the eyelid or even a slightly droopy eyelid. Should this happen, Iopidine eyedrops will counteract the effect until it resolves spontaneously in 7-21 days.

  • Some patients experience a slight headache that can be relieved with Paracetamol.

  • Slight bruising can occur, but can be concealed with make-up.

  • It resolves in a few days.

  • The effect will kick in after 7-10 days and last between 2 and 7 months, depending on the dosage used and the patients physiology.

Smokers lines or lipstick lines are fine wrinkles and lines on the upper and or lower lip that radiates out from the red lip border.

These lines are more prevalent on the upper lips.

  • Smokers lines or lipstick lines are fine wrinkles and lines on the upper and or lower lip that radiates out from the red lip border.

  • These lines are more prevalent on the upper lips.

  • While they are sometimes called smoker’s lines, most individuals develop the lines for reasons other than smoking.

  • Smokers lines are also known as perioral rhytides or “pucker lines”.

  • The “smokers lines” are worsened when the mouth is puckered by the contraction of the Orbicularis Oris muscle that surrounds the mouth like a purse string.

  • The lines are a result of loss of volume that occurs with age, along with repetitive bunching of the tissue from muscular activity.

  • These lines can contribute to lipstick bleeding, hence the name 'lipstick lines'.

  • The Orbicularis Oris muscle is highlighted in the image below.

Lipstick lines are usually treated with advanced dermal fillers, Anti Wrinkle Injections or a combination of both.  The depth of the lines and extent of puckering will influence treatment modalities.  It is very common to use Anti Wrinkle injections to treat lipstick lines regardless of the depth of the lines.  Treatment will have a couple of positive effects on the lines over time.  First, it will help relax the lines, which tends to eliminate the more permanent etching that may have occurred.  Secondly, it will help extend the longevity of the advanced dermal fillers.  Anti Wrinkle injections are generally placed at certain points in the orbicularis oris muscle which surrounds the mouth, including the area of the vermilion border of the lips. 

If the lines are a little deeper, then Dr. Kelleher will place a soft, smooth dermal filler along the lines to help fill them.  Also Dr. Kelleher may place a denser dermal filler underneath and horizontal to the lines to help elongate the area and stimulate collagen growth.  This will help with the puckered look, as well as diminishing the furrowed lines. 

Adding some bulk to the lips with dermal fillers will also have a positive effect on the lipstick lines by further elongating the lip that has contracted over the years.  Additionally, this treatment will help with lines on the lips themselves.


WHAT TO EXPECT IN YOUR TREATMENT

A lot of things may cause the condition:  small teeth, too much gum tissue, and a thin upper lip can each be a cause. In all cases of gummy smile, genes are at fault.

  • The condition known as “gummy smile,” when a person’s smile reveals far more of the gums than is considered attractive.

  • A lot of things may cause the condition: small teeth, too much gum tissue, and a thin upper lip can each be a cause. In all cases of gummy smile, genes are at fault.

  • The rule of thumb when smiling is two millimeters of gum showing is ideal. Anything over three to four millimeters of gum showing starts to look ‘gummy’.

  • About 7% of men and 14% of women have this type of smile.

  • Cosmetic surgery; either a crown lengthening procedure, which takes away gum tissue, or severing certain muscles that lift the upper lip, so that it reveals less of the gums; was the only option previously. This can be very expensive and painful.
